NEW PATIENT
INFORMATION

Prior to scheduling an appointment:

 

Before we can schedule your child's appointment, please be sure to have your child's doctor fax us a order for treatment for the requested service with a diagnosis code noted. Doctors may also use our Referral Form in lieu of writing their own prescription. We must have the prescription/referral form on file prior to scheduling an appointment.

If you are looking to transfer to Helping Hands from another therapy provider*, we will need the initial evaluation performed by the other therapy provider, as well as their latest re-evaluation/goal check, and a prescription/referral form from your child's doctor. Once this information is received, our lead therapists will review the outside documentation to see if we can bypass our new patient evaluation process to get your child right on the therapy schedule.

We accept many insurance policies (please see the list below of our current accepted insurances). Balances deemed patient responsibility per your insurance are due at the time of service. We accept major credit cards, cash, and personal checks.

Cancellation/Attendance Policy:

 

The Cancellation/Attendance Policy at Helping Hands can be reviewed in our new patient intake.

 

We charge a $50 cancellation fee for appointments cancelled without 24 hours' notice, and there is a $50 charge for "no call, no show" appointments.

 

If you cancel your child's appointment 3 weeks in a row, or "no call, no show" your child's appointment 2 weeks in a row, we reserve the right to remove your child from the schedule.

Regular attendance is essential for your child's growth in therapy. In order for us to serve all patients more efficiently, our office does NOT operate in coordination with public or private school schedules. Please call us or check our website for closings or delays. Our office is closed in observance of 6 holidays, and they are listed below:

  • Thanksgiving Day

  • Christmas Day
